Registration Fee Inclusions
The registration fee includes admittance to the forum, program materials, and all food functions as specified on the agenda.
Registration fee does not include hotel accommodations.
For meeting registration fees to be refunded, written notice of cancellation must be received by Hemedicus by 3/12/19. The amount of registration fees remitted minus a $75 administrative charge will be refunded.
Substitutions are permitted with written notification.
After 3/12/19 no refunds will be made.
